Private Candidate No-ID Form

Download and complete this form if you cannot provide an acceptable current passport, photocard driving licence or another approved form of photographic identification.

Before you begin

The form must be completed by the candidate and verified by an eligible independent professional who knows the candidate personally. A recent passport-style photograph must also be provided.

How to complete the process

  1. Download either the Word or PDF version of the No-ID Form below.
  2. Complete all sections relating to the candidate.
  3. Ask an eligible independent professional to complete and sign the verifier section.
  4. Include a recent passport-style photograph as instructed on the form.
  5. Check that every required section has been completed and signed.
  6. Email the completed form and photograph to Intuition Exam Centres.
